Jackson County Health Department immunization clinic:

COST: Childhood shots cost $15 for those without insurance, but are free for those unable to pay. Oregon Health Plan should cover the cost for children enrolled in that program.

WHAT TO BRING:  Please bring health records, including history of previous vaccinations.  If you have health insurance, please bring your insurance information.  If your child is on the Oregon Health Plan, bring his/her card.
