Birth Certificates

Q: How do I obtain a Birth Certificate?

A: You can obtain a birth certificate from the Jackson County Vital Records office up to six months from the date of birth. If older than six months you have to order the birth certificate from Oregon Vital Records, P. O. Box 14050, Portland, OR, 97293-0050. The fee for certified copy of the birth certificate is $20.00 for the first one ordered and $15.00 for any additional ordered at the same time.
